That's correct. In an oculomotor nerve palsy, "down and out" refers to the eye being pulled downward and laterally by two unopposed muscles: the superior oblique and the lateral rectus, innervated by the trochlear and abducens nerves. These are the only two muscles that would be functioning since all the other muscles are innervated by the oculomotor nerve. But is this consistent with the H-test in this case? Return to the case to reconsider. (note the dilated pupil on the right as well. What causes this?)
